Social Determinants of Health

Activities to address social determinants of health contribute to overall community and population health, and are part of important national and statewide delivery system and payment reform activities. GNYHA has developed tools and resources to support members as they implement social determinants of health screening tools, interventions, and related partnerships with community-based organizations.
