Vaccine Eligibility Expanded to 65+; NYS Vaccine Webinar Recording

The New York State Department of Health has expanded COVID-19 vaccine eligibility to include anyone age 65 and older. DOH’s updated guidance (attached), New York State’s COVID-19 website, and the New York State COVID-19 Vaccine Form reflect this change.

All vaccine recipients must complete the State’s form prior to vaccine administration. Individuals being vaccinated must bring proof of eligibility to the vaccination site, which may include an employee ID card, a letter from an employer or affiliated organization, or a pay stub, depending on the specific priority status. If eligible due to age, individuals should bring a form of ID that includes date of birth.
