The New York State Department of Health (DOH) will deploy a survey via the Health Emergency Response Data System (HERDS) to gather information on the usage and training of the Evacuation of Facilities in Disaster Systems (eFINDS) application. This survey will go to all hospitals and nursing homes in New York State.
This survey is expected go live on August 15 with a deadline of November 1.
Survey information is attached. Facilities should receive a notification through HERDS when the survey is available.
