GNYHA and its long term care affiliate, the Continuing Care Leadership Coalition (CCLC), developed The Colors of Safety Across the Continuum of Care initiative to improve and standardize patient-centered care across acute and long term care settings.

GNYHA with support from the United Hospital Fund seeks to improve the care transitions processes across all settings of care by identifying the factors that contribute to preventable hospital readmissions and effect solutions to avoid such readmissions.
