Medications are a key ingredient in improving the health of patients. However, when errors are associated with the use of medications, the consequences can result in signifi cant  morbidity and even mortality—which, in addition to being undesirable outcomes, can also be very costly to the hospital. To focus on important interventions and changes in processes for safe medication practices, GNYHA initiated the Medication Safety  Forum in early 2000. The Forum provides an opportunity for GNYHA members to share best practices in medication safety and for GNYHA to provide education and tools to assist members in the process of addressing medication safety issues at their institutions.
