Identifying Target Populations

  • Give these people influenza vaccine!
    Profiles high-risk groups by age, occupational and chronic illness that is indicated to receive influenza vaccine. (Immunization Action Coalition)
  • Double Check for Prevention
    Adult chart stickers to be placed on the front of medical charts to remind staff which patients are indicated for a dose of flu or pneumococcal vaccine. Note: Print on colored labels (NYC DOHMH)
